This subseries consists of responses to a questionnaire developed by the Department of Health, Education, and Welfare to gauge interest in refresher/reactivation programs. The questionnaire was sent to all nurses with inactive Georgia registrations. The files are organized alphabetically by county, and alphabetically by last name within each county.
