Scope and Contents of the Sub-Series

This series contains the files and papers of the fifth district's professional registry. The registry served to place private duty nurses where they were needed in an efficient and equitable manner. The registry had its own office, personnel, and financial independence, but was controlled by the private duty section and the registry Committee. Formed in 1928, the registry functioned well until the late 1960s, when it dissolved an a smaller community registry failed.

The registry files are divided into several discrete units:

  1. Sub-subseries 1: office files. This sub-subseries contains chronological files, 1934-1972, which contain office correspondence, policy manuals, and reports. In addition this section contains treasurer’s reports, 1941-1968, and auditor’s reports, 1958-1968 for the registry.
  2. Sub-subseries 2: Duty Log/nurse placement sheets. This series contains the day-to-day records of the registry's operations. The two components of this series, duty log notes (1958-1967) and nurse placement sheets (1963-1968) were used by the registrar to answer requests for private duty nurses and to keep track of which nurse was sent to which client and when. Each nurse working through the registrar had her own card which served as a record of her private duty career, and these cards are ordered alphabetically by last name.
  3. Sub-subseries 3: Physical examinations. This series contains physical exams for nurses working for the registry, dating from the late 1950 and 1960s, organized alphabetically. Note: these files contain personal information and are restricted. Restrictions are noted in the inventory.
  4. Sub-subseries 4: Letters of recommendation. This series contains letters of recommendation, alphabetized by last name, for nurses working for the registry or trying to gain membership, dating from the 1950s.
  5. Sub-subseries 5: Member history cards, 1931-1961: These cards, organized alphabetically, provide a record of each individual nurse. They include work history, education, and demographic information such as age and marital status.
